LS3.8.1- 'Failed to Add New User' -Error Msg.

Hi LS Experts,
Experiencing 2 issues related to 'add/create a new survey user.'
I had installed LS3.8.1 the other day & needed to add/create a new survey user.

Issue#1
However when I try to add a new survey user, LS does not generate the password email, instead LS generates a ‘failed to add user’ error msg.
After this when I go back & check under ‘Manage Survey Administrators’, I see that the new user is displayed as created.

Issue#2
Now when I try to reset this new users pw via the 'Forgot your password?' option, the User does not receive the LS email with reset PW.

But, I do see the new user as created/recorded in the "LIMESURVEY371.dbo.lime_users" table.

Any ideas on this? Have anyone experienced this?

This 'Failed to Add User' is still an open issue for me in LS381.
However a workaround to this R 2 Options:
1)Editing user (Admin resets user PW)
2)Forgot your password?(User re-sets their own PW)

This 'Failed to Add User' is still an open issue for me in LS381.

And it will always be. Bug fixes will only be applied to new versions. I am not saying that it is already fixed in 3.9.0, but it will never be fixed in 3.8.1, because this is basically impossible. To fix a bug a new version needs to be released and you need to update to this version to take advantage of new bug fixes.
By the way: I just tested today to add users in 3.9.0 and everything went smooth. The user received his login data, managed to login, managed to change the password in the system and managed to receive the reset password email.

Hi-
I had upgraded to LimeSurvey Version 3.12.1+180616 & I still experience this 'Failed to Add New User' -Error Msg.
Anyone else having this error type??

p.s- Unfortunate the only workaround is for Admin to use the 'Forgot your password?' route to get LS to send a temp mail to user & then user can login to LS to re-set PW.
