Multiple question types in array

Multiple question types in array was created by r.bushroot
I would like to create an array with multiple different question types (cf. attachment).
A work around has been suggested here and an example has been provided and disussed here . However, the scripts and examples are old and I could not make them run properly in my survey.
Could somebody provide my with instructions and potentially an example (LSS and script) that work with LimeSurvey 5 (5.3.21, own server)?

The shown example with two dropdowns in your second link should work in 5.x.

Thanks for the feedback.
I just found a potential explanation: To make it work, i.e., to make scripts work in questions, I need to deactivate the "Filter HTML for XSS". As this might not be the case, the script will not work.
Is this correct?
(I haven't been able to check it myself as I have not the necessary administrator privileges)

You definitely need to disable the XSS filter to place JavaScript in questions or groups. Did you try the simple alert as outlined in
manual.limesurvey.org/Workarounds:_Manip...tc..29_in_LimeSurvey
?
