It seems that the link is not saved in my question for some reason because when I return to the question to edit, the link I created is not there anymore (see Survey preview screenshot attached). How can I upload an audio/video file, so respondents can view it? Does the admin need to change the Global Setting before I can upload audio/video files? Thank you for responding.

Looks as if you still insert the <video> tags.

You are suggesting I am missing a step. If I am, could you indicate the step I am missing.

Tried changing video to audio using your code, but when I save the question, the code disappears and audio file is not shown to the respondent.

The only thing to do:
put the HTML-code into the source code of your question.
Make sure you enter the correct path.

And do you have rights to insert this?
Are you a superadmin or a regular user?
In this case ask the admin.
 

I did try entering the HTML-code. I think the problem might be related to admin rights. I will reach out to the admin. Thank you for you help!

Most probably the XSS-Filter is on and you are not a superadmin (the XSS filter doesn't impact superadmins).

So get in touch with the person responsible for your LS installation.
