Question with different types of sub-questions

Question:
I have a question structure which consists of many statements like for example:
"People feel safe from physical harm" 

Each statement/question then has three items to consider:
- Applicability (could be yes/no answer - preferably with optional comments box)
- Implementation (dropdown or similar single choice of 3 categorical variables Red, Amber, Green)
- Example (a text box)

There are 7 domains about 6 to 10 statements per domain, each statement needs 3 answers as in the above a structure. I have been thinking how to implement that with LS. Ideally the statement would be a question, then there would be three answers to it. Domains would translate to question groups. However the nearest I can see for now would be to create one question group per question, which would look pretty terrible (~60 question groups) and probably would be unwieldy too.

Is it possible to implement this in some more straightforward way?

as far as I understand, you would like to do something like this.




This is an array(text) with inserted drop-downs.
There are many, many examples here. in the forum.

As you see, the column widths are different. 
Also one of the "standard" workarounds.

Now, search the forum for this.
And send the lss export of your survey, that we can help better.

BTW: What about 60 groups? This is rather normal.
Many times recommended: One question per group and display "group by group".
This gives you the options to
  • exceptionally display more than one question on a page
  • merge questions either by css or by javascript (the most common use case)
And the respondent sees the pleasant "question by question" layout.
