I am trying to create a question with two types of answers ( boot strap plus text input). I am struggling to design it (I have attached the copy of the question here). Can someone help?

Thanks for your reply. Yes, that's what I tried. I used "single choice" bootstrap buttons. I am struggling to edit the options for "other"

..to add: if the response is 2 stays, I would then like to ask how many nights that they stayed in the hospital for each of those stays.

What do you mean by "options for 'others'"?
The text?
This you set in "Display".
Or the length of the field?
Investigate with the webdevelopment tool of your browser to find the affected classes (F12)

You can do this with some css
 
 

On the other side:
If you allow all numbers - more or less- I think it to be better you use a simple "numerical input" or a "multiple numerical input"
 

Yeah got that. For example - You have modified it to show "how many" - that's great. For that question, if someone responds "6"; then I want to ask them - for each stay - how many nights they spent at the hospital. Can we ask the question as shown in the paper survey (my first attachment)? or should we add that as a separate question?

1. An online survey is different from a paper-pencil-survey.

2. Of course you can do that.
Use the appropiate question type (multiple numerical input)

BUT: You used the wrong type of "bootstrap-buttons"
Use the "single punch" button!
   


And with this correct button the css is like this
You see that I used a custom css class "myOther".
This is to not affect other elements on the same page.

Here the survey (only group(s) MODRum and only the first filled.

I agree with Joffm, the correct approach would be a number question. The paper and pencil approach is not necessary online, it is not beneficial in any way.
