Do you have a screenshot? Sounds strange.
Also, which version of LS, which template/theme is used?
We need more information to work with.

Well, first of all, this is is a custom theme. So we really don't know what has been changed, etc. This will make things difficult.
Also, how does the problem look like in the backend? (Screenshot from the backend)
Error messages? Etc?
Does this happen an all MacOS and iOS devices or only on specific ones?
This is obviously a very, very specific error, because other spaces are recognized, while only this specific space does not show up on MacOS/iOS, but on Windows and Android. This sounds, very, very, very strange. At the moment, my guess would be that the space has been missing before, then was corrected and the Mac Device still has the old version in cache. Otherwise it would be difficult to explain that a space which is there (I assume you have double checked that and the tests on Windows and Android seem to confirm it is there), does not show only on Apple products.
(I don't think this is the problem here, but your version hasn't been updated for quite a while now, so you or better your university should consider an update to the latest version of the LS 3.x branch).

Any chance you can export a LSS file with just this section of the questionnaire? So that we can test here?
