I tried to upgrade from 2.70 to 3.0.2 and the upgrade failed with the following message

A non-recoverable error happened during the update. Error Details:
CDbCommand failed to execute the SQL statement: SQLSTATE[23000]: [Microsoft][ODBC Driver 13 for SQL Server][SQL Server]The CREATE UNIQUE INDEX statement terminated because a duplicate key was found for the object name 'dbo_lime_surveymenu' and the index name 'lime_surveymenu_name'. The duplicate key value is (<NULL>).

Current database version 328
Target database version 341

Any suggestions?
