I am trying to use a 10 point question with the following appearance. ( attached image )

I've got the shading working in css but the rest is resisting my efforts somewhat!

Replace the radio buttons with 1 - 10
Palce likert scale under question

Not exactly what you've depicted, but the array type question offers the "Semantic Differential" option - manual.limesurvey.org/Question_type_-_Array#General_description

Thanks but I'm aware of the standard options and that's barely close.

I'm trying to reproduce a clients existing look and feel.

In that case, you will need to use jQuery to insert those elements.
