New Install: Adding a Question Bombs back to the Home page

Hi, I'm sure there will be a simple answer to this but I am pulling my hair out...

I have a new install of Lime Survey that allows me to create a survey and a question groups. But when I then get to the next layer of the hierarchy, questions, it fails back to the list view of surveys.

I have tried setting debug to 1 (and even 2) in the config file and nothing seems to be reported (although esp. with it set to 2 the output is so huge I may be missing something - please direct me if I should look for something specific).

The PHP error_log file shows nothing.

Please help.

To me this sounds a little bit as if not all files have been uploaded completely.

How did you install LS? How did you upload files to your server?

I answer at the LimeSurvey forum in my spare time, I'm not a LimeSurvey GmbH employee.

Yes, I wondered that too, so I re-installed.
In both cases I used FTP from my PC. Is there a file inventory I can check against?

Did you make sure that your files were transfered as ASCII? Sometimes the FTP software transfers as binary and this can cause problems I think.

Let's see if someone else has another idea.

I think all the files are there.

What would really help is if there was some way to know what was failing. Like I say, setting debug=1 seems to reveal nothing useful. So is there a way to check what script is failing? Or missing?

what did you have in config for urlFormat ? If it's 'path' : try with 'get'

Yes I do have 'path' for urlFormat. But when I change to 'get' I cannot get past a page that says I have no surveys.

Has anyone ever had any issues for the "modsecurity" web server module causing problems like this?
