I need to create an answer template as shown in attachment, I am selecting the List(radio) button as Question type and inserting the HTML code in the answers section. I have following two problems:

1. The Radio Button is appearing along side the HTML table. Please refer to screen shot.
2. The answers are not recorded in the statistics.

The HTML code I used is a follows:

You can't just arbitrarily add radio buttons to the form.

You will need to define the buttons as answers in LimeSurvey and then use JavaScript/jQuery to generate the table HTML and move those buttons into the appropriate positions.

Thanks for your reply.

Could you please help me where I can find the information regarding the code to be written. I did try looking up in the manual but could not find it.


I am using the hosted LimeSurvey Professional (previously LimeService). I have not downloaded the limesurvey.
