Hello,

Think of using a "array 5 point" or "array"
Array 5 point : subquestion: & nbsp ;
Array: subquestion: & nbsp ; ; answer "Firstline<br />1","2" ....

Do you want to add such a line as shown at the screenshot or do you want to change the layout?

A "list radio" question type with advanced question setting "columns = 5" might alos be an option.

Do you want something that looks like this?



The dark square drags back and forth to choose the answer value



or perhaps something like in the post "9-point likert scale presented as a slid" (search forum for "slider" to find this post)
