Hi,
Have to say this is one of the best open source applications I have come across. I am using it as a web form, not a survey, and it works just as well.

My question: How to I change the format of the email notification template? I am editing the .printouttable style from the template CSS but it is not making any changes. Using view source on the email shows that the changes are not being applied to the email.

Thanks, but that was not what I was looking for.
I have had a comment that the email sent out with the answers is hard to read. I need to be able to edit the CSS of the email. You can see it below, (right click on the email), but I can't seem to change it. Changes I make to '.printouttable' in the style sheet are not being applied to the email?

No.. this css class set the {ANSWERTABLE} (to print the answers) and has no effect here...

What you can do easily is to allow HTML emails (show bottom of the sreenshot) and change your layout (for each survey) using the source mode of the editor

Nothing to touch into your template !
1) turn html email on (preferences settings of your survey)
2) edit the source of your confirmation email using HTML and embeded CSS around {LS_placeholders}
