Can someone give me an idea of how to do this? Also I need to figure out how to assign/score the responses to one of four categories. Will consider compensation for their time.
It looks like the dual scale is cleaner, but if I need to show two scales on one question, then I would need to know how to do that or get directed to the support page which shows how that is done.

Tony, wouldn't an array question do the trick?
Michael, if I get this right there are only two items to rate in total, one for the "most" and one for the "next"?

As stated above, the only solution I see is two array questions with an exclusion filter on the second question so the option selected in Q1 is hidden in Q2. You can place maximum and minimum answer settings to enforce a single answer in each array.
