I want every question have the same background and the same type of provision for answers and I'd put a picture for each answer . I tried to insert my image by sliding it into the text box of the answer but it sending me to another page with just my image.
And when images are the answers for , is it possible to click on the image to validate the answer?

You can't just slide images into the text box. You need to use HTML to include images into your survey.
