Data Protection Statement at beginning of the survey was created by tiflis

Hey everybody,

i have made a survey which contains sociodemographic items, so questions concerning age, sex and so on. How can i implement a data protection statement to the beginning of my survey, so that i have the respondents agreement at the beginning?

I am using:

Windows 7 Professional (and Windows 10 Home)
Mozilla Firefox Quantum 64.0.2
LimeSurvey Version 3.15.5+181115
