Is it possible to add empty fields to a 5-point matrix, which can then be rated from 1-5.

The simplest solution would be to ask a previous multiple-short-text question "Do you have any other foods you would like to rate?" and then place sub-question relevance on the array rows so they are shown when an answer is given in the previous question.
