Looks like a messed up / missing CSS. Can be a browser or server cache issue. Check with a different browser. If the result is the same, the LimeSurvey installation seems to have a some missing files or a messed up cache as well. Who did install this LimeSurvey installation? Any Updates done?

Hi Jelo,
maybe it was a server, or cache browser problem, i don't know. i've made a copy in local, tested (test was OK), and reupload and now everything is fine.

If you uploaded the files of LimeSurvey (e.g. via FTP or SFTP) a few files seemed to be not correctly uploaded in the first place. If you see such issues with web-applications it's mostly a file (e.g. css) which was not transferred correctly.
