This sounds like database querying/manipulation that is outside of the LimeSurvey capabilities. You will need to develop your own application to do this.This process will be implemented therefor we need a way to communicate with LimeSurvey concerning certain answers or certain statusses of a survey which invoke certain business events.

It sounds to me like gert wants to access a non-LimeSurvey DB. The API is only for LimeSurvey.The best seems to use the LimeSUrvey API , no ?

Push or Pull. A direct SQLquery from Limesurvey (Push) wouldn't be the preferred way in terms of security. But the Limesurvey API won't help in this case.
A pull from the medical database (or an external tool) might work via the Limesurvey API.
Depends on what gert needs and can use.

For the moment I'm looking into the possibility to ETL Survey data to our own datamodel so we can have the needed flexiblity to plugin an existing business process
