I could reproduce it.
Probably you got this error message when uploading the survey:
"Fehlende Umfrage-Dateien
Einige Umfrage-Dateien konnten nicht ..."
with the parameters of the file-upload question.
If you answer "Continue?" with "No"
you see a red icon on the interface.
Click and follow the instructions. (more or less: submit error message and survey url to support@offlinesurveys.com)

The standard file upload is disabled... We don't have a single use case to support it (we can't find real use to uploading files on an offline survey.) If you have a valid use case, please share it with us and we will try to support it accordingly.

But I cannot add a title to the foto although I enabled this option...?

It would be helpful to provide a title to the foto, because the collected data in the survey refers to retail shops. In order to assign the data to the taken fotos that would be necessary, as there is no access to the files from LimeSurvey responses page (it says file was not found). Or am I missing something?
