Multiple Short Text Defined width for each input box - Help was created by haroldrio

Hello everyone.

I would like to ask for help.
I am using limesurvey 2.50.
It is for multiple short text type

I wanted do something like this

I guess a java script would do the job, but unfortunately I don't know how to make the code for this one.

I think you will need to use separate questions as you show a textarea (long text) in the first row and text inputs (short text) in the other rows.
