Urgent Help required - response not being saved to database

I have a 27 question survey which appears to work fine. When a respondent completes the questions and submits, they get a thank you page to say the response has been submitted.
When I check the responses in the Control Panel/Dashboard, it says all the responses are 'Incomplete' and NONE of the answers are recorded in the database.
Using Limesurvey 1.92+ Build 120530.

This survey needs to be send to 10,000 people next week so can someone please HELP?

Does the Show Logic File show any errors?

Are you using a custom template?

Have you reviewed the actual database (e.g. using phpmyadmin) in case it is the control panel that is being misleading?

If you can attach the actual survey, that is the best way for someone to help.

No syntax errors in Logic File. (I've attached a pdf)

I've checked the db using phpmyadmin and all answers are null.

Interesting I have the same issue utilizing the latest 1.92 iteration and the 2.00 RC2.

There is a problem with one question

it seems to allow multiple selections in a subquestion

it also allows me to submit even thou i have not filled out many mandatory questions.
