Array with 3 scales, first two scales radio and third -drop down/open text

Dear experts,

I have a questionnaire that has three array scales. I could create the array with the help of the code I found from the previous posts. However, my third response scale has 11 categories, radio buttons it taking up more space. I was wondering if anyone could suggest a method to convert the third scale to drop down or open text.
Also, can anyone suggest the method where I can add a subquestion titled 'others' to this array at the end that provides an opportunity for the respondents to type in if they want to add any new concept?
I would greatly appreciate if anyone could help, the image the scale I now have is attached along with how the 'others' subquestion looks.

Hi, though I think this approach is the best, because in my opinion there should be a filter on the two first questions.
If "they never refuse to brush their teeth" the next two questions are obsolete.
If "this behaviour is not a problem" the third question is obsolete.

In an online questionnaire it's always better to ask question by question with a valid routing.

Nevertheless, I assume you want something like this.


Just merge the two workarounds for "dropdowns in matrix" and "matrix with others", which are described many times here.

By the way: You may add more others just by adding more "Other questions" and adding the subquestions.

Dear Joffm,

Thanks again for the prompt response. The lss file you attached has been quite helpful. I could get my questions structured the way I wanted. Thanks for the suggestion in relation to filters but I prefer not to have filters for several other reasons.
