I need a Likert-Scale looking somewhat like shown in the attachment. It's basically a "Dual Matrix" but with 3 instead of only 2 parts. (How) can this be done?

Thanks Joffm, this already helps a lot. However, I only get a dropdown-list with numbers 1-10 and can't seem to configure it (easily).
Instead of numbers (1-4), I'd prefer text such as "++,+,-,--"
If I understand one of your links correctly, this can be done with some coding such as:
