Hi,
it's very ambitious to implement such a form in LS. Always remember: online data collection is different from "paper/pencil" data collection.
I am sure you read the manual about the available question types.

So, one question before about the "document title": Will it be predefined, or is it an empty field where the respondent enters something?
How many different titles per respondent do you expect?

IMO the best way to achieve this is to create as many question groups as the maximum amout of titles/respondent, e.g. you show 20 textboxes for the titles. So you create 20 groups.

Each group contains the questions
Question (B): multiple
Question (C): date; (maybe the workaround in the manual about range sliders works)
Question (D): array

First question: Multiple short text: Enter the titles.
For each not empty text show the group by relevance and pipe the entered title into the question text.
Rest of groups are hidden by relevance.

Try and if there are further questions don't hesitate to ask.

Hi,

here is a rough working example, how I think you could realize your survey with the LS standard.

You can enter up to 5 titles, but only the first three titles are asked about later.

But you see the idea.
Up to you to copy the groups

Of course you can improve by css or so
