I am currently dealing with following issue:
for my survey I want users to be able to add additional answers to some questions, in case of none of the proposed/given answers fit.
I would offer a form, labeled as "other" in which the user can type in his/her answer. Then the user is able to add multiple answers.
These will only be visible for the specifc user who add them.
All questions and answers must be defined before the survey is activated. I think you will need to add multiple text questions for these "Other" answers and possibly hide/show them with conditions or relevance.
