I have successful installed LimeSurvey 2.5 in Windows Server but Survey Page don't display properly as attached. Any advice ?

Old templates might not work on 2.5. Try using the system default template for your survey.

IMHO it's better to send us a sample survey (*.lss).
And of course always the same question: Which version of LS do you use?

You have a simple "single response question" and you use the default template, as you say.
So, there shouildn't be any issue with that.
If somebody can reproduce your issue, then there should be a bug tracker.

Maybe something is wrong with your installation or something missing.

The survey looks normal on my server. Can you activate a live version and give us a link so we can see the source?

I just find the solution. My php version not compatible with LS 2.5 . . Thank you everybody for your assistance.
