Invitation email erroneously not being sent to user

Invitation email erroneously not being sent to user was created by tzadiko
Hello,
I'm trying to send an invitation email to the last user in this table:
For some reason, I get the following error message:
As you can see from the table, the user has a valid email address.
The user was never previously sent an invitation.
The user not yet completed the survey.
The user has an access code.
The user has one use left.
Yet the email is not sending.
Why is this?
Furthermore, if I wanted to send the access token in the email, what would I need to put in curly braces?
E.g. {FIRSTNAME} for first name. Is there a page that summarizes this?

Thanks for your response, but now I receive the email all messed up.
I noticed this: www.limesurvey.org/forum/installation-a-...-showing-up-in-email
But I cannot find those settings. Before I click on the "Send Reminders" the result I see is HTML.
