Can someone explain to me how I can set-up the following in "one" question?

Expected completion date: ____ /_____ /_____
End date unknown (ongoing): -Radio Button-

The simplest approach would be to create 3 separate questions and use CSS to make them appear as one. The CSS would vary depending on the template used.
