Multiple Choice with checkbox and free-text box

Hi Everyone

I am very new to LS so apologies if this has been asked a million time in a million different ways but I can't find the solution I'm looking for

What I'm trying to do is build a survey which has


Checkbox - Selection - Free-Text

Example:

Question: How many times have you visited these facilities in the last X no. of months

Tick - Visited the canteen - 5
Tick - Visited the top floor - 3
Untick - Visited the car park

I really don't have any idea how to format it as I tried the multiple choice with comments which seemed to be on the right path but I don't know how to create the selections against each checkbox to allow the users to make their selection and add their numeric free-text response at the end of that row

Any help would be massively appreciated
