Having "N" group of questions for each item

I'd like to know how to create a dynamic survey based on user numerical input. i.e for each n items, how yo have n group of questions?

For example, imagine this question

How many TVs do you have? [enter a number > 0]

>> 3 TVs

TV number 1:

- Question 1
- Question 2
- Question 3: "How many antennas are there?"

>> 1 antenna

Question 3_a: Antenna number 1:

-- Question 3_a1
-- Question 3_a2

TV number 2:

- Question 1
- Question 2
- Question 3: "How many antennas are there?"

TV number 3:

- Question 1
- Question 2
- Question 3: "How many antennas are there?"

There is no "dynamic" creation of groups or questions. You need to create the maximum allowed groups and hide/show them with relevance.
