My site is reporting that visitors are trying to access /favicon.ico (which is returning a 404 error). I see that favicon.ico is a file in the /limesurvey/admin directory. Does anyone know how to fix this problem or why it would be happening?

if you look at the url bar on your browser on this site you will see an icon on the left side, which is a small lime icon. That is what favicon.ico is for limesurvey.org.
Obviously all browsers will look for the file, if it doesn't exist then it will just be ignored, so its no big deal. If you want an icon then put one in your site root.

For purely cosmetic reasons I would like my site to have a special Favicon in the URL bar ... I substituted the favicon.ico file with our logo but it still only shows the default Internet Explorer icon.
Can't work out why - all the other (Joomla) sites we have display the specified icon

Have you replaced the favicon.ico file with a .ico file or are you trying to call a gif or jpg for example. It must be a .ico file

Yes - it is a .ico file .... what I will do is try remaking the icon file and see what happens.

Take care because those filenames can be case sensitive.
Check the file size, too. Maybe the upload did work. Another option is changing the upload method (binary).
