I have 30 plans that we want folks to vote on. Each plan is a page long, and we want participants to vote 0-5 (likert scale) for each of them. I know I can create a 30 page survey, with one plan on each page with voting.

Is it possible have all of the plan titles with voting for each of them on one page, and have each title hyperlinked to the details of each plan? This way voters can quickly, in one place, on each plan, and if they need to refer to the details of a plan, they can jump to the details of a plan and back again.

Is this possible?

Also, is there anyway to make a 5pt likert scale look like 5 Stars?

1) You can create links to anything you like in the question editor. Just make sure you have them open in a new page (target="_blank").

No, you cannot link directly to a different group while taking the survey.
