We want to be able to match IP-addresses gathered in surveys to specific locations (County, city, and / or state) and generate reports based on those locations. Are there any plugins / scripts available for LimeSurvey that can help us do this?

The tag words should be "geo location" or "geo targeting". There should be lists that can match (to a certain extent - in some cases ISPs are not very exact in this) IPs to locations. Most of them you probably will have to pay for.

You need the IP to country mapping info to get your result. If you are looking for free mapping service, below are a few link you may consider:

1. www.ipinfodb.com
2. www.fraudlabs.com (free license)

if you are looking for more accurate data, i will recommend: www.ip2location.com

Yep and for ipinfodb, you have sole javascript example (with ip2country lite)
www.ipinfodb.com/ip_location_api_json.php

You can use your own database (dowload one here : www.maxmind.com/app/geoip_country) or some php script github.com/maxmind/geoip-api-php .

And make an ajax call to fill a question with country from $_SERVER.
