I have a feeling this a really dumb question... but I tried searching the manual and had no luck.

I thought entering {SURVEYNAME} in to one of the questions would indeed put in the name of my survey... but it hasn't. Is there something else I should be trying?

Seems that placeholder isn't available in questions. Try adding the following to the source where you want to display the name.
<script type="text/javascript">document.write($('title').text());</script>
