What is LimeSurvey ?

LimeSurvey is a leading Open Source Online Survey Tool written in PHP. 

LimeSurvey allows you to develop, publish and collect responses to surveys. LimeSurvey includes a wide range of built-in question types, and a range of flexible options.

Your surveys can include branching, your own preferred layout and design (using a templating system), and can provide basic statistical analysis of survey results. 

Surveys can be public, or can be strictly controlled through the use of "once-only" tokens for each survey participant.

What can it be used for?

Jason Cleeland: "I work for a labour union, and we use surveys of various sorts quite regularly for a range of reasons. Surveys are an important organising tool, and they can be invaluable for a range of things - from getting information regarding the sort of things members want from Enterprise Bargaining negotiations, to keeping in touch with members views on current working conditions. We've used paper based surveys for a long, long time, and we've never had a good way to centralise the administration of them, or to simplify the issuing of surveys. A script like this - particularly the 'token' feature - can really increase the usefulness of surveys, while at the same time reducing the administrative workload. We now use LimeSurvey (formerly PHPSurveyor) for more than just traditional surveys, including registration for training courses, and simple promotional questionairres."
