Limesurvey is a fantastic tool and I have come to depend on it heavily. I am not sure if somebody has already posted this requirement.

I want to use limesurvey 1.85 as a form for a particular patient record. There are 2 aspects to this form. One is static "one-time" data and the other is dynamic "on - visit" data.

Static one time data:
1. Name 2. Age 3. Address 4. Unique ID Number

Dynamic "on-visit" data
1. Visit Number 2. Temperature 3. Blood Pressure etc.

I need to search for a patient record and update only the dynamic data. I will search for the patient record on name and a unique ID. I need to update this with the outcome of every visit.
Needless to say, I need ALL the features of limesurvey, especially question conditions.

Is this possible to do?

Some thoughts:

1. I vote for using tokens to store the static data.

2. Your suurvey should be non-anonymous.

3. Take a look at the setting "enable token based answer persistance".

4. Use the token as unique ID.

5. I think there is a workaround to use one token multiple times, check manual -> workarounds.

Thanks for the quick response. I think I should write a small workflow or a requirements document so that I can describe the full breadth of what I am trying to do. I will do so in the near future. I may want to use your premium offer to do this. I will keep in touch thro' a private email.

I'm looking forward to hearing from you. It might be quite tricky to set this up according to your needs but with some workarounds I think Limesurvey can be used for this.
