Now every time I start a survey it is asking for a language on each question screen.

I know how to make the language picker go away, but there is a blank question screen before the first question and I don't know how to get rid of it.
The administrator has disabled public write access.

Please switch on debug mode by adding the following line to your config.php file:
$debug = '1';

Are there any additional errors reported on top of the page?
