I have designed a token based survey
I have tested this before on the same server and php_mail worked fine
but now, there seems to be a problem
I contacted the hosting and they say everything is fine
but there are no email sent to me mailbox

I am using:

LS 1.90+ Build 9071
Hosting: 000webhost.com (free, but supports php_mail)

Also please let me know, how php_mail can send emails via some admin mailbox like:

without having the password for it?

Try using a different email method. This can be set at global settings -> email.
