Whenever I create this type of question, not only do I get the subquestion with the 10 choices, but it also creates an additional version of the same subquestion with a free text box below it. Any idea how to set it so that this additional version won't show up?\

I should also add, this only happens when I 'test' or 'activate' the survey. If I do a 'preview', the additional version does not show up.
