I have seen several question sets that have a low end statement then a number rating then a high end statement. I would like to upload a pdf, however seems that functionality is currently broken...

Thank you for your suggestion, however, my audience is of an age and technical/dexterity challenged era that would have a difficulty with sliders.

Have a look at the array question, and at the semantic diferential in specific in the documentation:
docs.limesurvey.org/Question+type+-+Array

If you separate your two statementes with "|" they should appear as you wish.
