How can I add a single checkbox, and which is already ticked?

Another option (if difficult) is to use a yes/no radio, where yes is pre-chosen.

Hello

If you have a >1.90 version, the pre-checked answers are availables for some types of question like button radio, dropdown menu etc.
Set your question and look for the following icon to choose your default value:
