How or can I add question type "upload file" in survey like that in bug and tracker / blow of this form?

There is no question type which allows users to upload a file.

You can implement this but it won't be easy. Lots of changes have to be made (export, startistics, ...).

Another solution is to auto-redirect users to an upload form after they have finished the survey. You can try to pass the token they used and name the uploaded file using the passed token.

Or if you're not using tokens on the survey, but still want some tracking capabilities, use a combination of timestamp and IP address -- not perfect, but unless you have high volume, should be fine. Just be sure to disable anonymous survey in your survey setup.
